Abstract

Moving into a care home can be viewed by the older person as the 'end of the line' after failure to attain a positive outcome following a rehabilitative process. Yet there are many situations where an older person's condition improves following admission to a home, despite no formal rehabilitation. While this suggests that rehabilitation potential can be missed and active rehabilitation should continue beyond the move, it also highlights the importance of supporting residents through the transition from active rehabilitation to long-term care.
